Abstract


The authors have observed a case of lichen planus occured in molar portion of the buccal mucosa in 42 year-old woman.
The results are as follows:
1) The lesions appeared on bilateral sides of the buccal mucosa near by the interocclusal lines of molar portion, and revealed typical Wickham¢¥s striae.
2) Histopathologically, the keratinized and parakeratinized layer revealed irregular and
tniam zassive ker ll ni, and the keratohyaline granules were contained not only in the
granular cells but also in the parakeratinized layer. The prickle and basal cell layer revealed
moderate intercellular hydropic degeneration with intercellular infiltration of lymphocytes,
but revealed neither intracellular hydropic degeneration* nor intracellular inflammatory
cellular infiltration. The basal cell layer was destroyed and/or separated from the underly-
ing connective tissue, and could see the formation of eosinophilic thin band immediately
subjacent to the degenerated basal cell layer. The band of chronic inflammatory cellular
infiltration consisting primarily of lymphocytes representing in the papillary and subpapil-
lary layer was sharply demarcated from the deep portion of connective tissue.
3) Histochemically, intercellular substance of the prickle cell layer revealed slightly PAS-
positive reaction, but did not reveal the met¢¥achromasia by the stain of toluidine blue.
4) The basement membrane represented thickening and intensive PAS-positive reaction.
5) The mast cells appeared being scattering in the papillary and subpapillary layer.
